When to use this tool
Use this template for a specific action and a seven-day review window. Not recorded remains unknown instead of becoming not done. The count describes the entries; it does not explain every reason behind them or measure effectiveness. Keep a note when your action, schedule, or recording process changes.
Connect it to the decision
Use the original example to understand the three states, then review what helped and one adjustment. The printable sheet, CSV, and workbook let you keep a simple record without relying on a provider’s account or reminder features.

Questions to resolve
- Do my worksheet answers become site content?
No. The tool uses your entries in browser memory. Use the explicit print or export action if you want a copy, and reset to clear the workspace.
- What does a blank amount or answer mean?
Blank means not entered or unknown. Use zero only when that is the actual amount you intend; the calculation asks for required inputs instead of silently assuming them.
